Apply to the CFC

Thank you for your interest in AWF and the 2013 Combined Federal Campaign.

To make your application process run smoothly, we recommend gathering the following documents prior to downloading the 2013 CFC Application Form:

• IRS Tax Exemption Letter 
• IRS Form 990 (most recent version covering a period ending on or after 6/30/2011).
• Audited Financial Statement (dated on or after 6/30/2011) required only if revenue reported on Form 990 exceeds$250,000
  

NOTE: The Form 990 and audited financial statements must cover the same fiscal period).

If the Audit or Form 990 are not available, you may still complete the application form and submit the financial documents later.
